Local support for Young Carers and Young Adult Carers
The Bridge Young Carers Service - Family Action work with Young Carers and Young Adult Carers (5-24), and their families. Support includes:
- Young carers assessment and follow up support plan
- One to one sessions
- Peer support groups
- Access to Carers breaks

Help when someone is being discharged from hospital
Carers will be given one to one support from one of a specialist Hospital Discharge Team at Durham County Carers for up to 4 weeks and after that by the wider team.
